At the end of the presentation, participants will be able to:
- Define and classify the various stages of chronic kidney disease (CKD)
- Identify the risk factors for progression from CKD to end
stage renal disease (ESRD) - Identify opportunities for timely referrals and co-management
strategies with nephrology - Apply appropriate clinical measures to aid in slowing the
progression of CKD
