Position Description

JOB DESCRIPTION

Our Palliative Care providers manage chronic debilitating illnesses and end of life issues. Provide cross coverage as needed/requested by nursing and medical staff. Round on assigned medical hospitalist patients and on patients as requested by facility medical staff.

Core responsibilities:

  • Conducts family meetings.
  • Performs consults and follow up visits as needed
  • Create, direct, coordinate, and supervise the patient care plans daily
  • Conduct/order diagnostic tests, and analyze results and prescribe indicated treatments and therapies
  • Round on existing hospitalist patients in multidisciplinary rounds if asked
  • Work as part of an integrated team with care management, nursing, pharmacy, etc.
  • Assist in patient education and goal setting
  • Participate and adhere to all quality metrics and goals as established and directed by Adfinitas or the client/hospital
  • Submit accurate and timely charges for all professional services performed
  • Attend committee, CQI and management meetings, as appropriate

  • Highly effective verbal and written communication skills, including the use of email, are necessary in dealing with a variety of healthcare professionals, Adfinitas colleagues, family members, and patients
  • Team oriented attitude toward patient care at facility including care managers, hospitalists and the nursing staff
  • Demonstrate commitment to high ethical standards and healthcare compliance
  • Skill in diagnosing and treating diseases
  • Ability to effectively work in stressful situations

REQUIREMENTS

  • 1+ years of experience as an Advanced Practice Provider in an acute role, preferably long-term care or skilled nursing facility or similar inpatient experience required
  • Graduate of an accredited school and program, respective to role (PA-C, or NP)
  • Licensure to practice medicine in good standing, with the state governing body
  • Federal DEA certification in good standing
  • State CDS in good standing
  • Possess strong clinical skills and demonstrate competency in providing comprehensive health care including evaluation, diagnosis and treatment
  • Flexibility and organizational skills to work autonomously
  • Proven skills, knowledge base, and judgment necessary for independent clinical decision making
