Nurse Practitioner New

Pride Health is hiring a Nurse Practitioner for long-term care to support our client’s medical facility based in Camden, New Jersey. This contract opportunity is a great way to start working with a top-tier healthcare organization!

Responsibilities:

• Perform comprehensive assessments to evaluate patients’ medical, behavioral, and psychosocial health and create personalized care plans.
• Manage complex conditions by diagnosing and treating acute and chronic medical and behavioral health issues to ensure continuity of care.
• Maintain accurate documentation of patient information, including ICD-10 codes, to support care planning and compliance.
• Foster team collaboration by working closely with interdisciplinary care teams to deliver coordinated and patient-centered care.
• Facilitate advanced care planning by conducting discussions on advanced illness and end-of-life care to help patients and families make informed decisions.
• Educate and support patients by providing counseling and guidance on disease management, preventative care, and long-term health goals.

Qualifications:

• Graduate of an accredited MSN/DNP (Nurse Practitioner) or PA degree program.
• Active certification as an NP (AANP/ANCC) or PA (NCCPA).
• Current DEA license with prescriptive authority.
• Basic Life Support (BLS) certification.
• Full COVID-19 vaccination.
• Reliable transportation.

Additional Information:

• Location: Burlington, Camden, and Mercer counties, New Jersey
• Travel Requirements: Travel between approximately 4 facilities during the shift.
• Job Type: Contract – 22 weeks – *possibility of extension*
• Schedule: Monday to Friday – 08:00 am to 05:00 pm
• Local Pay Range: $ 60.00 to 70.00/ hour

*Offered pay rate will be based on education, experience, and healthcare credentials.
