Ideal candidates will be:

• Currently licensed to practice professional nursing or Physician Assistant in the state of Pennsylvania, active and non-probationary
• Experienced in primary care: internal medicine, geriatrics, or family medicine
• Current BLS/ACLS certification
• Driven to and comfortable with managing a primarily Medicare-aged, medically underserved patient population with comorbid conditions

Job Duties include:

• Patient assessment, diagnosis, and education
• Treatment and management
• Care coordination
• Health promotion and disease prevention
