Medix is seeking a compassionate and motivated Nurse Practitioner (NP) to join a fast-growing, innovative healthcare organization that is redefining how primary care is delivered to seniors. This is a direct hire position at a clinic focused on value-based care in a welcoming, community-centered environment.

Our model emphasizes quality over quantity, giving providers more time with each patient (45-minute appointments!) and the support they need to offer truly holistic care. Key Responsibilities

• Provide comprehensive outpatient primary care to a Medicare-aged population
• Work in collaboration with physicians and clinical teams to deliver coordinated, relationship-based care
• Spend 45 minutes with new patients to fully understand their needs and develop long-term care plans
• Manage chronic conditions and focus on preventative care to reduce unnecessary ER visits
• Perform in-person and occasional telehealth visits (e.g., in the event of no-shows)
• Participate in daily team huddles and end-of-day administrative time
• Help monitor and track patient outcomes, follow-ups, and preventative screenings

Schedule

• 8:00 AM – 5:00 PM
• Full time, M-F

Requirements

• Active, unrestricted Nurse Practitioner license in Pennsylvania (or eligibility)
• Background in Primary Care, Internal Medicine, Family Medicine, or Geriatrics
• BLS/ACLS certified
• Comfortable treating a primarily Medicare-aged population with chronic conditions
• Strong clinical judgment, diagnostic abilities, and a team-focused mindset
• Willingness to travel locally between centers (if needed)
