For over 20 years, the Visiting Physician Services has been bringing PRIMARY CARE into the comfort and privacy of your home. Now a part of the As apart of the VNA Health Group, this team of primary care clinicians continues to grow.

Visiting Physician Services specializes in treating adult and geriatric homebound patients. We make house calls to private residences, senior housing buildings and assisted living facilities in Ocean, Monmouth, Middlesex, Somerset, Union, Essex, Passaic, and Bergen Counties.

Our practice is comprised of physicians, nurse practitioners and physician assistants with the mission of treating elderly, frail, homebound patients with dignity and compassion .

We are currently looking for a Nurse Practitioner to join our team to cover sections of Monmouth County, such as Freehold. The position would be responsible for ensuring delivery of high quality care practices in the treatment of symptoms and promoting the highest quality of life for homebound patients and their families.

Responsibilities

  • Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
  • Assesses, diagnoses and develops appropriate plan of care to treat homebound patients
  • Monitors the patient’s condition and responsiveness to the treatment provided
  • Acts as an agency representative with patients and their families as well as with other health agencies, groups and organizations
  • Adheres to agency infection control and safety policies
  • Adheres to the organization’s policy in regards to absenteeism and appearance
  • Omission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this position

Qualifications

  • Master’s degree in Nursing required from an NLN accredited program
  • Licensure as an Advanced Practice Nurse (APN) in NJ with prescriptive authority; maintain a CDS and DEA
  • National certification as an Adult or Geriatric Nurse Practitioner
  • 2+ years of APN experience and/or significant experience as a registered nurse
  • Excellent communication, organizational and interpersonal skills
  • Strong computer skills

Working Conditions/Physical Demand: Working indoors/outdoors with Client/Customer contact and clinical equipment and use of phone and computer; valid driver’s license and automobile insurance.

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $118,580.00 to USD $296,131.88. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
